Membership Applications & Related Forms

Please click the pdf link for your required form. Print, fill out and return the form(s) to the CHCA office at the address on the form. Allow ample time for mail. Forms not recieved in time may result in your need to fill them out again on race weekend!. Most are required only once a year.

Entry forms are time sensitive regarding fee, please fill them out early.

For Everyone

GENERAL MEMBERSHIP
Required to be on file for each member including drivers/riders, co-drivers and workers. Optonal for crew members. (yearly)

WAIVER
Must be signed and accompany each Membership. (yearly)

For Competitors

ENTRY FORM
Competitor race entry. Must inclue information on co drivers if applicable. *One form for both drivers only. (Requied Per Race)

MEDICAL FORM
Required for all drivers/riders and co drivers, filed before "first run". (yearly)

W-9 TAX FORM
Required; naming payee and filed before "first run". (yearly)

For Parents of a Minor

*Minors are those children under the age of eighteen (18) at the time of their first race weekend.

MINOR PARTICPATION PACKAGE
All forms required to be on file for parents who enter a minor child. (yearly)
